A high red blood cell count looks alarming on a report, but the explanation is often simple. Here you learn the ranges we use and the common causes, sorted from everyday to rare. You also see which patterns in your blood count point to a false alarm and which need a doctor.

What does a high red blood cell count mean?

A high red blood cell count means there are more red cells in each liter of your blood than the reference range expects. Most often your total number of red cells has not grown; the blood is simply more concentrated because it holds less fluid. Less often, the body is making extra red cells.

Three values on a complete blood count describe the same cells. Red blood cells (RBC) are the count. Hemoglobin is the oxygen-carrying protein inside them. Hematocrit is the share of blood volume the red cells take up. That is why they usually rise and fall together.

StatPearls sorts high values into three groups. Relative polycythemia means less plasma, not more red cells. Secondary polycythemia means the body makes more red cells in response to a signal, such as low oxygen. Primary polycythemia means the bone marrow makes too many on its own.

What are normal red blood cell, hemoglobin and hematocrit values?

In our knowledge base, a red cell count from 3.94 to 5.71 ×10¹²/L and a hematocrit from 0.348 to 0.50 L/L are within range. For hemoglobin, the app uses the WHO 2024 cut-offs, which only define the low end. Your lab may print separate ranges for men and women.

Sex matters a lot here. StatPearls gives typical adult ranges of 13 to 18 g/dL hemoglobin and 40 to 54% hematocrit for men. For women it gives 12 to 16 g/dL and 36 to 48%. So a hematocrit of 50% sits above the usual female range but inside the male one. A single combined range can miss that, which is why your sex belongs on the report.

What causes high hemoglobin and hematocrit?

The most common causes of high hemoglobin and hematocrit are dehydration, smoking and living at high altitude. Testosterone or anabolic steroid use is another frequent reason. Lung and heart disease, some tumors and polycythemia vera are less common but more important to rule out.

Relative (less fluid in the blood). Dehydration concentrates the blood without adding red cells. StatPearls also describes a stress-related form (Gaisböck syndrome) seen with high blood pressure, smoking, obesity and water tablets (diuretics).

Secondary (the body makes more red cells). StatPearls lists smoking, high altitude, chronic obstructive pulmonary disease (COPD) and obesity hypoventilation syndrome. It also lists tumors that release erythropoietin (EPO), the hormone that drives red cell production. The StatPearls blood count review adds heart disease that lowers oxygen levels and kidney disease. The WHO 2024 guideline adjusts hemoglobin cut-offs for smoking and for altitude, because both raise hemoglobin.

Primary (the marrow overproduces). Polycythemia vera is a bone marrow disorder. StatPearls notes a JAK2 gene change in about 95% of cases, usually with a low EPO level. It states that a hematocrit above 49% in men or 48% in women at sea level is suggestive of polycythemia vera.

Can testosterone therapy raise hemoglobin and hematocrit?

Yes. StatPearls lists testosterone replacement therapy, anabolic steroids and EPO-like drugs among the medical causes of a high red cell count. If you use any of these, a rise in hemoglobin and hematocrit is an expected effect to watch, not a surprise.

Could a high result be a false alarm?

Often, yes. If you drank little before the blood draw, had a stomach bug, or trained hard in the heat, your blood can be more concentrated for a while. A repeat test when you are well hydrated often brings the values back into range.

Which patterns point to which cause?

The pattern across the whole blood count says more than any single high value. The table lists common combinations and what usually clarifies them. It prepares you for the talk with your doctor and is not a diagnosis.

The fourth row is a common trap. A report can show a high red cell count together with small cells and low hemoglobin. That is not "too much blood". See a doctor soon if your hematocrit stays above 0.50 L/L (50%) on a repeat test without dehydration. The same applies if a high value comes with headache, dizziness or brief episodes of blurred vision. StatPearls names these symptoms in polycythemia, along with a higher risk of clots in arteries and veins. Call emergency services for chest pain, sudden breathlessness, sudden weakness or numbness on one side, or trouble speaking.

If your sex is missing, the app asks for it, because the same value can be normal for a man and high for a woman. For a high hematocrit or hemoglobin, the suggested re-test is after about 8 weeks; for red cell count, about 16 weeks. Drink water as usual before that draw. The app explains patterns in plain words and does not diagnose polycythemia.
